Death Certificate Requirements


Death Certificate from the National Statistics Office (NSO) is an official document setting forth particulars relating to a dead person, including the name of the individual, the date of birth and the date of death.
An NSO death certificate is a record issued by a government office like the National Statistics Office that declares the date and location of a person's death.
When requesting for an NSO death certificate, the interested party shall provide the following information to facilitate verification and issuance of certification.
Death Certificate: 
  1. Complete name of the deceased person
  2. Date of death
  3. Place of death
  4. Place of marriage
  5. Complete name and address of the requesting party
  6. Number of copies needed
  7. Purpose of the certification
